Why did the greatest massacres of all time happen at the hands of the State?

Governments are institutions optimized for harboring the most sophisticatedly evil people.

Why were Hitler and Stalin heads of State, as opposed to truckers or plumbers or loggers?

Because in the State, the winner is the man who can lie best and be the most ruthless — most professions don't optimize for that.

Add to that the fact that most people reflexively obey or worship the State, and you get to the result: you can do unspeakable evils for a long time and maybe never face reprisal for your actions.
